Job Description:
Glenview Terrace - JOB SUMMARY: The purpose of this position is
to plan, organize, develop, implement and interpret the programs,
goals, objectives, policies and procedures, etc. of the Wound Care
Program

Professional
- - - - - - - Must be a graduate of an accredited school of nursing
and hold a current RN / LPN license in the State of Illinois.
- - - - - - - Must have working knowledge of nursing services,
nursing administration, rehabilitation, and general nursing, and
geriatric nursing.
- - - - - - - Must be qualified to assume the position by training
and / or previous experience of a supervisory status, preferably in
a health care facility.
- - - - - - - Minimum of two years experience as a wound care nurse
preferred with a strong clinical background.
- - - - - - - Certification as a wound care nurse preferred.
- - - - - - - Basic computer skills.

residents and public and act as an example to employees.

FUNCTIONS:

- - - - - - - - - Support the facility's philosophy of care and
strive to achieve its goals and objectives.

- - - - - - - - - Directing wound care assessments and treatment
interventions to all patients.

- - - - - - - - - Responsible for evaluating all residents with
wounds identified upon admission.

- - - - - - - - - Provide patient, family, and staff education on
appropriate wound care techniques.

- - - - - - - - - Responsible for ensuring compliance with state,
federal, and JCAHO standards related to wound care.

- - - - - - - - - Collaborates with managers and unit staff on
identifying and implementing appropriate wound care practices.
-
- - - - - - - - - Responsible for planning, development,
coordination, implementation and evaluation of the facility wide
wound management program.
-
- - - - - - - - - Provides consultation services to health care
team regarding the management of pressure ulcers and complex
wounds.
-
- - - - - - - - - Participates in the discharge planning of the
patient with pressure ulcers or complex wounds.
-
- - - - - - - - - Applies research based interventions and
documents outcome data for interventions.

- - - - - - - - - Participates in the development and provision of
formal / informal education programs for the health care team
related to pressure ulcers and complex wounds.

- - - - - - - - - Assists in the collection of performance
improvement data to include prevalence and the incident data
related to pressure ulcers and complex wounds and the promotion of
tracking / trending within the facility.

- - - - - - - - - Conducts weekly rounds with the wound care
physician to assist in the treatment and documentation of the wound
status and treatment plan.

- - - - - - - - - Other duties includes; Foley catheter care,
feeding tube stoma care, Colostomy / Ileostomy care, PICC line
dressing changes, Documentation on treatment records, keeping carts
clean, stocked and organized, preventative measure such as heel
protectors, mattress, wound cultures with f/u with lab and m.d.,
all skin care treatments.

- - - - - - - - - Maintain the confidentiality of resident
information and honor their personal and property rights.

- - - - - - - - - Follow established fire, disaster, and evacuation
policies and procedures.

- - - - - - - - - Attend weekly Medicare/PAN meeting,
nutrition/wound meeting.
- - - - - - - - - Ensure care plans are up-to-date as needed.
- - - - - - - - - Ensure families are updated on a regular basis
regarding wound progress.
- - - - - - - - - Ensure proper documentation of treatment nurses
(admission assessments, weekly wound assessments, facility pressure
injury documentation and risk management, Braden scores, signing
TAR etc..)
- - - - - - - - - Manage inventory of treatment supplies, place
order weekly, coordinate with part B supplier.
- - - - - - - - - Perform regular in-service for staff regarding
pressure injury prevention, skin assessment, wound identification
by floor nurse.
- - - - - - - - - Weekly wound rounds with physicians.
- - - - - - - - - Coordinate with facility PCPs on status of wound
care patients.
- - - - - - - - - Coordinated with social services for patients
with wound/ostomy needs upon discharge (NPWT, treatment supplies,
ostomy supplies etc.
- - - - - - - - - Perform other related duties as directed by the
Administrator and DON.
