Job Description:

Ascension is seeking a dedicated and compassionate RN-Case Manager to join our team in enhancing patient care based on our faith-based principles. In this role, you will leverage your nursing skills to manage patient cases across our network of hospitals and clinics, ensuring the continuity of care and effective resource management. Conduct comprehensive patient assessments to determine their medical, emotional, and social needs. Develop, implement, and evaluate individualized patient care plans in collaboration with other healthcare professionals. Advocate for patients' needs and provide guidance and counseling to patients and their families on treatment options. Coordinate with healthcare providers to streamline care transitions, reduce hospital readmissions, and ensure treatment adherence. Monitor patient progress and make necessary adjustments to the care plan based on evaluation and feedback. Ensure compliance with healthcare regulations and internal policies in managing patient cases. Maintain accurate documentation of all patient interactions and interventions. Strong analytical and problem-solving skills Effective communication and interpersonal skills Proficient in case management software and medical records systems Ability to work autonomously and collaboratively in a fast-paced environment Knowledge of healthcare regulations and policies Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of practice Certification in Case Management (CCM) preferred but not required
