Job Description Participates in provider on-call rotation as required. Minimum of 1 year experience working with the frail or elderly population is required. Board Certified in Internal Medicine or Family Medicine - REQUIRED Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED IN license (or willing to obtain) - REQUIRED Clean Malpractice - HIGHLY PREFERRED CDS Certification (by time of start) - REQUIRED DEA Verbal Confirmation (by time of start) - REQUIRED BLS (by time of start) - REQUIRED Minimum of 1 year experience working with the frail or elderly population (add to "Experience" field) - REQUIRED Previous supervisory experience (add to "Experience" field) - REQUIRED Does the provider require visa support - J1 or H1B?
