Occupational Therapist - School Location: Rockford, IL Agency: Bluebix Health Inc Pay: $52 to $62 per hour Contract Duration: 13 Weeks Start Date: ASAP About the Position We are seeking a dedicated Occupational Therapist (OT) for a 13-week contract in a Home-Based Primary Care setting in Rockford, Illinois. This role involves providing patient-centered care in a home health environment, helping patients improve daily living and functional independence. Responsibilities: Evaluate patients and develop individualized treatment plans Provide therapy to improve daily living and functional skills Educate patients and families on adaptive techniques and equipment Monitor patient progress and adjust care plans as needed Maintain accurate and timely documentation Travel to patient homes and deliver quality care Collaborate with physicians and healthcare teams Requirements Minimum 1 year of experience as an Occupational Therapist Home health experience required Active Illinois OT license BLS certification (AHA) Strong communication and patient care skills Benefits Competitive pay (negotiable) Guaranteed 40 hours/week Mileage reimbursement Weekly pay Overtime opportunities Opportunity to work with a reputable healthcare facility
