A. - - - Personal:
1. - - - - - - - - A broad general education.
2. - - - - - - - - Evidence of good grooming, appropriate manner and conduct.

4. - - - - - - - - Integrity, imagination, initiative, energy and evidence of self-improvement.
5. - - - - - - - - Participation in professional organizations and workshops and continuing education.

1. - - - - - Certified Nursing Assistant by the state of Illinois
2. - - - - - Must be on the Illinois State Nursing Aide Registry with a clear record

2. - - - - - Physical appearance: Must present a pleasing image to residents and public and act as an example to employees.

A. - - - - - Support the facility's philosophy of care and strive to achieve its goals and objectives.

B. - - - - - - Responsible for the maintenance of rehab supplies and equipment.
-
C. - - - - - - Cares for and applies or supervises the application of orthoses, braces, prostheses, and any other appliance or assistive device.
-
D. - - - - - Provides hand on support to implement the specific individually designed rehab programs.
-
E. - - - - - - Documents daily participation and completes monthly progress notes as needed.
-
F. - - - - - - - Monitors walk and dine program and other rehab programs.
-
G. - - - - - Demonstrates knowledge of resident safety and abuse prevention programs.
-
H. - - - - - Recognizes and reports any changes in resident's condition to the charge nurse.
-
I. - - - - - - - - - Participates as a team player.
-
J. - - - - - - - - Answers call light promptly.
-
K. - - - - - Attends and participates in care plan meetings.
-
L. - - - - - - Assists in serving and feeding residents.
-
M. - - - - Assists families and visitors when appropriate.
-
N. - - - - - Follow established fire, disaster, and evacuation policies and procedures.
-
O. - - - - - Perform the duties of CNA's as needed for patient care. -
-
P. - - - - - - - Perform all other duties as directed by the Director of Rehab.
